In today’s clinical environment many patients seek evaluation in the primary care office for shoulder pain. The shoulder is, however, a complex joint and as such represents a challenge to those who are less comfortable with shoulder anatomy or the complexity of the special test used in the diagnosis of shoulder injury. With the time constraints we face daily due to ever shrinking appointment times the skills needed for a good musculoskeletal (MSK) exam such as the shoulder become lost with a quick, and sometimes unnecessary, referral to orthopedics. With a little refresher, however, we can more confidently make the diagnosis and provide better quality care for our patients.

This presentation will cover the role of Physical therapy in pelvic floor dysfunction and expectations for evaluation and treatment. Pelvic floor dysfunction negatively impacts quality of life, however over half of symptomatic patients do not seek medical treatment. Patients may dismiss the idea of Pelvic Floor Physical Therapy due to their discomfort with exams, feelings of embarrassment, or lack of understanding of what to expect on the day of their appointment. Physicians' comfortability discussing Pelvic Floor Physical Therapy expectations can decrease patients’ anxiety regarding treatment and improve patient compliance with treatment and attendance.

Acute ankle sprains are the most common lower limb injury in athletes and account for 16%-40% of all sports-related injuries. It is especially common in basketball, American football, and soccer. The majority of sprains affect the lateral ligaments, particularly the anterior talofibular ligament. Despite its high prevalence, a high proportion of patients experience persistent residual symptoms and injury recurrence. A detailed history and proper physical examination are diagnostic cornerstones.

In the Primary Care outpatient setting, physicians will encounter patients with symptoms that may be related to sleep disorders. A significant portion of the population suffers from both insomnia and hypersomnia. Physicians may not have the tools or knowledge to properly evaluate these patients and perform a proper diagnostic workup. Physicians may not feel comfortable with prescribing a controlled substance as treatment related to sleep to patients and knowing about non-pharmaceutical treatments would be helpful to manage these patients. This presentation will cover many of these concerns and should improve care of patients.

The American Association for the Study of Liver Disease (AASLD) states that MASLD affects over 30% of the global population and is now the primary cause of chronic liver disease. Metabolic dysfunction-associated steatotic liver disease (MASLD) is a major driver of hepatic disease and is associated with numerous conditions seen in primary care, such as prediabetes, type 2 diabetes, obesity, atherosclerosis, and dyslipidemia. Hepatic disease associated with MASLD exists on a spectrum and can progress to liver fibrosis, cirrhosis, and even hepatocellular carcinoma. Nomenclature of this condition, previously known as non-alcoholic fatty liver disease (NAFLD), has recently been revised.  This presentation will review the updated terminology, as well as review guidelines and recommendations that exist to help with screening, identification, monitoring, referral indications, and treatment of this disease.
