A significant portion of our population lives with chronic conditions. Diabetes is one of the major chronic conditions we a osteopathic physicians manage. Performance measures for chronic conditions such as diabetes have been in development for over a decade and are now being used to guide not only how we manage diabetes, but how we are evaluated in our efforts as clinicians in improving the health of patients with diabetes. This presentation will discuss the development of quality measures, how they are being used, and what osteopathic physicians should know about how we are being measured in our management of this chronic disease. At the same time, some diabetes clinical "toolkits" will be presented to help physicians achieve quality measurement goals
